We offer individuals with disabilities a meaningful and integrated activities through two different programs, Day Services and Pathway Skills Center. Community Learning Services (CLS G) is also integrated in both programs.
Focus on programs for individuals ranging in age, skill level, support needs, and activity preference. Activities include participation in basic life skills, socialization, social responsibility, and other skills that build independence.
Pathways Skills Center:
Focus of program is for individuals aged 18 - 35 with varying abilities and skill levels. Programs that support individuals transition from young adults to adulthood. Pathways provide activities that emphasize life skills training, education and pre-vocational skill development, socialization, and social responsibility.
PERSONAL ASSISTANCE/ HABILITATION (PAB)
One to one services provided in the individuals own home or family home. The individual receives support to develop, retain, and improve skills, related to living in their home. Activities include daily living skills, light housework, meal preparation, and many other instrumental daily activities.
RESIDENTIAL HABILITATION (RESHAB)
Provides oversight and monitoring of in home care services for individuals who reside in state licensed adult foster and care homes.
COMMUNITY LEARNING SERVICES-INDIVIDUAL (CLS I)
One to one services provided in the community. The individual receives support to integrate and access community resources, increase safety awareness, develop social and communication skills, and gain experience that builds independence and social responsibility.
Temporary residential service coordination on a short term basis.
